This notice details and applies to the practices of Elite Plastic Surgery and the practices followed by all of Elite Plastic Surgery workforce members who handle your medical information.
Elite Plastic Surgery acknowledges that your medical information is personal and Elite Plastic Surgery is committed to protecting it. Elite Plastic Surgery maintains records and conducts treatments with aim of giving the highest level of protection to your medical and personal information, while still giving you the highest level of medical care. This notice applies to all records of your medical care received or created by Elite Plastic Surgery.

The ways in which Elite Plastic Surgery may use and disclose medical information regarding you is detailed in this notice. Your medical information, also referred to as “protected health information,” includes your demographic information, or any other information that could identify you and that relates to your past, present or future physical or mental health and related health care services.

Uses and disclosures to others involved in your healthcare

In the event you are unable to agree or object to this disclosure, we may disclose such information as necessary if we determine that it is in your best interests based on our professional judgment.
Your protected health information may also be used or disclosed by us to notify or assist in notifying a family member, personal representative or any other person responsible for your care of your location, general condition, or death.

If at any time emergency treatment is needed, we may provide your protected health information to necessary parties. In this event we will attempt to obtain your permission and acknowledgement of this event where and when possible, which may occur after the treatment has been administered.

Time to time, we may use and disclose protected health information to inform you about certain health related benefits or services that may be of interest to you.
Uses and disclosures required by law
Your protected health information may be used or disclosed by Elite Plastic Surgery when required to do so by federal, state, or local law. This will be made in compliance with the law and will be limited to the relevant requirements of the law. If the law requires us to do so you will be notified of any such uses or disclosures. Your protected health information may be disclosed to a public health authority authorized by law to receive reports of child abuse or neglect. Moreover, we may disclose your protected health information if we believe that you have been a victim of abuse, neglect or domestic violence to a governmental entity or agency authorized to receive such information. Disclosures for Workers’ Compensation

Right to inspect and copy

Should you request a copy of the information, we may charge a fee for the costs of copying, mailing or other supplies associated with your request. Requests to inspect and copy your information may only be denied in very limited circumstances and you have a right to request that any such denial be reviewed.
Right to request restrictions
You have the right to request that Elite Plastic Surgery restrict the use and disclosure of your protected health information for treatment, payment and health care operations. However, Elite Plastic Surgery are not required to agree to your request. We reserve the right to change this notice. Elite Plastic Surgery also reserves the right to make the revised or changed Notice effective for protected health information Elite Plastic Surgery already has regarding you, as well as any information Elite Plastic Surgery may create or receive in the future. Complaints
You may file a complaint with Dr. Rozbeh Torabi and Dr. Radbeh Torabi Office Manager or with the Secretary of the Department of Health and Human Services in the event you believe your privacy rights have been violated and/or that Elite Plastic Surgery or Dr. Rozbeh Torabi and Dr. Radbeh Torabi has not followed this policy.

Other uses of protected health information
Your written permission (“authorization”) is required for other uses and disclosures of your protected health information not covered by this notice or the laws that apply to Dr. Rozbeh Torabi and Dr. Radbeh Torabi. Should you provide us permission to use or disclose protected health information regarding you, you may revoke that permission, in writing, at any time. Should you revoke your permission, Elite Plastic Surgery will no longer use or disclose protected health information regarding you for the reasons covered by your authorization. Please understand that Elite Plastic Surgery is unable to take back any disclosures Elite Plastic Surgery may have already made with your permission, and that Elite Plastic Surgery is required to retain records of the medical treatment or other services that Elite Plastic Surgery provided to you.
